K-8 2023-2024 SCHOOL YEAR ENROLLMENT APPLICATION & AIA PHYSICAL EXAM/CLEARANCE FORMS.

Below is a list of documents needed to start enrollment for the 2023-2024 SY. 

If you are unable to print, stop by our K-8 Office for a package.

New Students (Kindergarten)

  • Must be 5 by December 31, 2023
  • Completed Enrollment Application
  • Certificate of Indian Blood for Student
  • Birth Certificate for Student.
  • Current immunization dated 2023 or later - no handwritten immunization will be accepted.
  • If applicable - IEP
  • If applicable - Guardianship or legal/court documents.

Transfer Students

  • Completed Enrollment Application
  • Certificate of Indian Blood for Student
  • Birth Certificate for Student.
  • Current immunization dated 2023 or later - no handwritten immunization will be accepted.
  • Last School Year Report Card.
  • Withdrawal Form.
  • If applicable - IEP
  • If applicable - Guardianship or legal/court documents.

Plan on joining sports?

The 2023-2024 AIA Physical Exam and Clearance is now available.  

Avoid missing school days and practice time!  Plan ahead and get your physical exam and clearance done this summer.   Having your physical done every summer gets you cleared for the school year.
