New and returning student enrollment forms for 2024-2025 school year will be ready for download soon.  If you do not have access to printing, please stop by one of our front offices to pick-up a package. We look forward to meeting our new families and welcoming back those who are returning.

Shonto Preparatory Schools accepts enrollment application packets for students who range in age from five (5) years old through eighteen (18) years old. Student must be five (5) years old by December 31 st of the current school year to be eligible for enrollment.  Attendance, behavior, and academic checks will be completed for students who wish to attend SPS.

In addition, we also provide dormitory (residential) for students grades 1st through 8th.  Click here for more information regarding our dorm life and their services.

NEW Student Enrollment

This enrollment process should be completed for students who will be NEW to the either K-8 or High School.

Required Documents:

• Completed Enrollment Package;

• Your child's Birth Certificate;

• Your child's Certificate of Indian Blood;

• Your child's Current Immunization | Arizona State Vaccine Requirements.

Additional Documents:

• Guardian Documentation (if applicable);

• Withdrawal from previous school with last report card/official transcript;

• Arizona Proof Of Residency for High School Students;

• Incoming Freshman - Copy of Promotion Certificate;

• Exceptional Educational Services Documents (IEP) (if applicable).

• In anticipation of Athletics for the 2024-2025 SY, we encourage students to complete the required Physical Clearance/Examination provided by the AIA.

Kindergarten

• Children are eligible to enroll in kindergarten if they are five years old by December 31 of the current school year.

• Due to classroom capacity, enrollment for Kindergarten may close sooner than deadline.

RETURNING Students

• Returning students will need to update their enrollment application if there are changes in contact information or if student relocated.

• Students will also need to update their Health History Questionnaire & Consent Form yearly.

• Students will be notified by our school nurse if an updated immunization is needed.
